Why do riser ESDVs fail?

HSE have investigated 170 reported incidents where hydrocarbon pipeline riser valves have ‘failed’ – in most cases these were either failure to close on demand, or within the specified time. Their research report covers both immediate and root causes, and concludes that too often duty holders just fix the fault(s) without really investigating why they occurred. So that results in repeated failures – 45% had failed previously, some up to 3 times – and thus increased risk to persons on the installation.

The report provides some useful generic templates for investigating root causes of incidents, though HSE warn that the ‘maintenance’ causes need to be extended to specifically cover ESDVs.
