UK sector rope access fatality

An offshore services company has been fined for serious safety failings following an incident in which a worker died after plunging 23 metres from a platform into the sea. A summary of the accident has been published by HSE, following a court case nearly 4 years after the worker died.

It is worth noting that the task planning breached IRATA Guidelines, and also that no action was taken by HSE against the platform operator.
