HSE's offshore statistics for 2015

The latest annual data from HSE covers both the range of accidental events reported to them and a summary of their inspections and other enforcement activities. There is useful comparative data for previous years, from which no very obvious general trends can be discerned, except a fairly steady reduction in major and significant hydrocarbon releases since 2010.

By far the greatest proportion of the data is for oil & gas activities, though it’s notable that one of the two successful prosecutions was for a fatality during maintenance on a windfarm.
