I am an Inspector of Marine Accidents and I served in the
Merchant Navy as an engineer officer with the Royal Fleet Auxiliary
for 24 years.
To help explain our powers and procedures, I will use
video/audio clips of extracts from cases that I have
worked on myself over the previous 5 years .
Investigations that I have led in my five years as an inspector
include:
2012 High speed passenger craft Moon
Clipper – Contact with London City Pier on the River Thames,
resulting in injuries to 14 passengers and 2 crew.
Container feeder vessel Clonlee - Electrical blackout and
subsequent grounding on the River Tyne
2011 RO-Ro ferry Oscar Wilde - Machinery space
fire
2010 Container vessel Ever Elite -
Uncontrolled descent of an accommodation ladder while the vessel
was in San Francisco Bay, resulting in one fatality
2009 High speed craft Stena Voyager - The
shift and loss overboard of an articulated road tanker in Loch
Ryan
Passenger RIB Celtic Pioneer – Spinal injury to a passenger
during thrill ride in the Bristol Channel
2008 Fishing vessels Lady Hamilton of Helford
and Blithe Spirit - Collision in Falmouth Bay, resulting in
foundering of Lady Hamilton of Helford
Privately owned RIB Partner 1 - Console detachment in Studland
Bay, Poole
Army Cadet Rigid Raider - Capsize of Army Cadet Force Rigid
Raiding Craft in Loch Carnan, South Uist resulting in one
fatality
